One dossier refresh and one payer request, from the same binder
October 7. Veltrimab, an IL-23 p19 antagonist, was approved for ulcerative colitis on September 30, and Priya Raman is taking its Approved Product Dossier from v5.2 to v6.0. That morning new data arrives, and a request from Crestline Health Plan, received the evening before, is waiting for its P&T meeting. Here is what happens, screen by screen, in the working solution.
- 01Morning
The whole dossier, and how fresh each section is
Priya Raman · US payer value communication, dossier ownerThe binder lists the 17 AMCP Format 5.0 sections of the v6.0 draft — 90.7 pages, sign-off target October 24 — each with its owner, pages, the sources it rests on and when it was last updated. Four are current and signed, three were reviewed with no change, nine are being updated, and §4.2B Cost-effectiveness analysis waits for the UC report due October 20. The binder was checked against 412 library sources at 09:14.
“Point at a source to see the sections that rest on it.”
- 02New evidence
Medical Affairs sends the week-100 abstract
Priya Raman · dossier ownerThe VLT-302 extension week-100 results arrive from Medical Affairs as a congress abstract: two pages, one table and one Kaplan–Meier figure. Priya adds it to the evidence library with one button, “Add and check the binder”. The agents read it and check every binder section that rests on it.
- 03Seconds later
Three sections turn amber
Evidence Mapper & Change Tracker§1.1B Clinical benefits, §3.1B Study summaries and §3.2B Evidence tables all rest on VLT-302 and were last updated before today, so each is marked out of date. Their freshness tracks show the new source as a tick past the last update, and the library feed shows the abstract with “3 out of date”. The library now holds 413 sources.
“3 sections now rest on evidence newer than their last update.”
- 04One click
Three cited updates, sent for medical review
Section Writer“Draft the 3 updates” adds the new data to each section: of patients in clinical remission at week 52, 78.4% were still in remission at week 100 (as observed, n=208). Each value carries its citation, and the three sections go to Omar Haddad for review. The references list gains one entry.
The week-100 data is labelled “as observed” — the section writer keeps to what the source says.
- 05Section by section
A redline against v5.2, with the reason for every change
Priya Raman · dossier owner§2.1B Product information shows each change with its reason — UC approval or new WAC — and its source. The indications now include ulcerative colitis, the UC dosing (600 mg IV at weeks 0, 4 and 8, then 200 mg subcutaneously every 8 weeks) and the hepatotoxicity warning are added, and the WAC reads $19,240 per 100 mg syringe, $19,860 from January 1, 2027. The section is ready to sign; 7 of 17 sections are done.
“Each change shows why — click the reason to see its source.”
- 06Two readings
A value that differs between two places goes to a person
Table & Figure ReaderIn §3.2B Evidence tables, every value shows where it was read — a table or a figure — and whether it was confirmed. For week-52 endoscopic improvement the abstract says 50.1%, while Figure 3 and its source data in Table S4 say 50.2% (136 of 271). The reader recommends Table S4 because it holds the patient counts; Priya decides, and the decision goes into the change log.
Limit: no value from a figure without a confirming table or sentence.

Crestline Health Plan asks four things
Request Intake & Unsolicited CheckerDr. Hannah Brooks, PharmD, sent REQ-2417 through the eDossier portal on October 6 for Crestline's November 12 P&T meeting — 1.4M lives. The binder maps each ask to the sections that answer it: A1 the whole dossier, A2 §4.3B and the Excel model preset to 1,400,000 members, A3 the two subgroup rows of §3.2B as a 3-page excerpt. A4, adolescents aged 12–17, is not answered from the binder.
A4: “Not answered from the binder: an unapproved use (label §8.4) · Medical Information replies.”
- 08Before anything is built
The unsolicited check, the edition and the budget model
Priya Raman · dossier ownerThe request letter sits beside the answers, with the unsolicited check: it came in unprompted through the portal, the requester sits on the P&T committee, and there was no company contact in the 30 days before. Priya sends v6.0, since its October 24 sign-off falls before the October 29 packet deadline. The budget impact model, preset to Crestline, shows +$0.031 per member per month in year 1, rising to +$0.087 in year 3, with every input left editable.
Unsolicited check: “No company contact in the 30 days before.”
- 09Ready to send
Four files, nothing unrequested, two approvals
Packet & Deck Builder, then MLRThe packet holds a cover letter that answers A1–A3 one by one and says A4 is answered separately, the v6.0 dossier (released when it is signed), the Crestline budget model and the evidence excerpt. The payer deck stays out, because Crestline did not ask for slides. Priya hands A4 to Dana Okafor in Medical Information, then Omar Haddad and Lena Ortiz approve, and the packet can go through the portal ahead of the October 29 deadline.
“Payer deck left out. Crestline did not ask for slides, so the deck stays with Sam Patel for the account meeting.”
