In short

What is Hub Enrollment Form Processing?

It is the work of turning a specialty patient's enrollment form — faxed, e-signed or sent through a portal — into a complete patient-support case: splitting the packet, reading the fields, checking signatures and consent, asking the office for what is missing and opening the case for benefits verification. Hub Enrollment Form Processing gives that work to six agents, and a hub intake lead approves every request and case.

The problem

Why a specialty patient waits a day or more for the hub to open a case

A specialty enrollment starts with a manufacturer form: two or three pages the patient signs for the programme authorization and text consent, and the prescriber signs for the prescription, faxed to the hub with insurance cards, a demographic sheet and a chart note. Hub intake splits the packet, keys about 31 fields into the hub system, checks the form against the brand's rules, faxes the office for whatever is missing and only then opens the case for benefits verification.

The gap is usually found last. An intake specialist keys the whole form to learn the one signature is missing, and every gap is a fax round trip to the office. Until the patient authorization is signed and dated, no health information should flow to the programme and nobody should call the patient — and a form that ticks ulcerative colitis but carries a Crohn's code, or a member ID that differs from the card, stalls benefits work later.

How it works

How a form moves

Six agents read every form against its page and the office’s usual gaps and draft the fax-back; the intake lead approves each request and case.

One enrollment form, from the fax line to an open case

At 07:42 a six-page fax arrives at Northwind Patient Support from Lakeshore Gastroenterology in Chicago: a Veltrimab enrollment for Marisol Vega, ulcerative colitis, prescribed by Dr. Elise Novak. Here is what happens next, screen by screen, in the working solution.

  1. 0107:42

The fax arrives — and its usual gap is checked first

Dana Okafor · Intake team lead

Dana's Incoming forms screen shows each arrival on fax line 2 with its office, time and status. Lakeshore sent 64 forms in 30 days and only 48% arrived complete, against 58% for the network. Its usual gap is the patient authorization, unsigned on 21 of 64 — so before the agents read anything else, they look at that box.

“Checked first on every Lakeshore form. The form is usually faxed before the patient signs at check-out.”

  2. 02Seconds later

Six pages, 31 fields, every value tied to its box

    Packet Sorter and Form Reader

The Packet Sorter splits the fax into the enrollment form (pages 1 and 2), the prescription, the demographic sheet, the insurance card and the chart note. The Form Reader reads 31 typed and handwritten fields into the patient record, each with its confidence and page; click a row and its box lights on the faxed page. Three were read below 90%.

Every value keeps its page and box — the Form Reader never infers a value that is not written.

  3. 03The verdict

The authorization is blank — boxed in red, outreach locked

    Completeness Checker

Twelve of the 15 Veltrimab form rules pass. The patient signature and date in section 2 are both empty, so the rule that blocks the case fails and the line is boxed in red on the page and on the record. Patient outreach stays locked until the authorization is signed; the text-consent box is ticked.

“Until it is signed, no health information goes to the programme and nobody contacts Marisol.”

  4. 04Already drafted

A fax-back to Rosa Diaz, asking only for what is missing

    Office Request Writer

The request to Rosa Diaz, Lakeshore's office coordinator, at (312) 555-0148 asks for the patient's signature and date on the authorization — page 1, section 2 — with the page attached and the line marked. It reminds the office to black out Social Security numbers: the one on the demographic sheet was masked on arrival. It cannot go yet.

“2 items on this form need your decision before it goes — ICD-10 code does not match the box; member ID differs from the card.”

  5. 05Two decisions

A Crohn's code on a UC form, and two swapped digits

Dana Okafor · Intake team lead

The ulcerative colitis box is ticked, but K50.90 — a Crohn's disease code — is written; the prescription line and the chart note both say ulcerative colitis. The member ID reads NWH 4417 2093 on the form and NWH 4471 2093 on the printed card, read at 99%. Dana asks the office to confirm its code and takes the card value.

“This is a form check, not a clinical call — the office confirms its own code.”

  6. 06Approved

The request now has two items, and nothing more

Dana Okafor · Intake team lead

The Office Request Writer adds the code question: “Please confirm the ICD-10 code. The form ticks ulcerative colitis, but the code written, K50.90, is a Crohn's disease code — page 2, section 4.” Thirteen of 15 checks now pass. Dana reads the letter, can edit the wording, and approves and sends it.

“Attached: page 1 with the lines to complete marked. No insurance numbers or diagnosis codes beyond what is asked.”

  7. 07Sent

Pending info, with the follow-up already scheduled

    Case Opener

The request is faxed to Lakeshore and the case opens in the hub system as Pending info. The follow-up timers start: a call to the office at 24 hours, a second request with the e-signature link at 48 hours, and close as incomplete at day 10. Outreach to Marisol stays locked.

“Faxed to (312) 555-0148 at 07:43. Patient outreach stays locked until the signed page comes back.”

  8. 08The reply

The signed page comes back and matches itself to the case

    Packet Sorter

Lakeshore's one-page reply is matched to ENR-24817 on two identifiers — the office fax number and the patient's date of birth, never on name alone. Marisol has signed and dated the authorization, and Dr. Novak confirms ulcerative colitis, K51.90: “K50.90 was written in error.” All 15 checks pass, and the hub may now contact Marisol by phone and text.

“Complete — confirm 2 fields. The office's reply completed every check.”

  9. 09Case opened

Two handwritten fields confirmed, then handed to benefits

    Dana Okafor and Sam Patel

Dana confirms the two handwritten values read below 90% — the date of diagnosis and the TB test — and opens the case. The Case Opener writes the cited fields to the hub system and hands it to Sam Patel, the case manager, who starts benefits verification with Harborline Health Plan.

“Opened in the hub system and handed to Sam Patel for benefits verification.”

  10. 10Fixing the cause

A coaching note so Lakeshore's next form arrives signed

Dana Okafor · Intake team lead

Lakeshore's page shows what it misses over 30 days: the patient authorization 21 times, the member ID 6, the ICD-10 code 4, the insurance cards 2. The Office Request Writer drafts a short note to Rosa from those gaps — have the patient sign section 2 at check-in, attach both sides of the cards, check the code against the box — with a one-page checklist. Dana approves it before it goes.

“Most gaps come from the patient signature on page 1 — the form is often faxed before the patient signs at check-out.”

Built on the engine

6 agents. Each with one job, and hard limits.

Six agents read every form against its page and the office’s usual gaps and draft the fax-back; the intake lead approves each request and case.

Packet Sorter

Splits each inbound fax or upload into its documents — enrollment form, prescription, demographic sheet, insurance card, chart note — and matches an office's reply to the open case.

  • Never matches a patient on name alone — a reply needs two identifiers
  • An unreadable page goes to a person
Form Reader

Reads every typed and handwritten field on the brand's enrollment and prescription form into the case, with the page, section and box for every value and a confidence.

  • Never infers a value that is not written
  • Below the confidence threshold, a person confirms
  • Social Security numbers are masked on arrival and never stored
Insurance Card Reader

Reads payer, member ID, group, Rx BIN, PCN and Rx group from the front and back of the card and compares them with what the form says.

  • Never overwrites the form value silently — it shows both to the intake specialist
Completeness Checker

Runs the 12 to 15 rules of each brand form version — required fields, signatures, consent, the ICD-10 code belongs to the box ticked, the NPI check digit, the form version — checks the office's usual gap first and gives a verdict with reasons.

  • No clinical judgement — only the form against its own rules
  • Every finding cites its page and box
Office Request Writer

Drafts the missing-information fax to the office in plain words, listing only what is missing or needs confirming with its page and section, and attaches the page to sign with the line marked. Also drafts office coaching notes.

  • A person approves every request before it is sent
  • Minimum necessary patient data — no clinical advice, no full insurance numbers
Case Opener

Opens or updates the case in the hub system with every confirmed field and its citation, sets it to Pending info or ready for benefits check, starts the follow-up timers and assigns the case manager.

  • Opens a case only after a person approves
  • Patient outreach stays locked until the authorization is signed
Intake specialist

Confirms and approves. The agents propose; a named person decides.

Ask in plain words

Ask about any form, office or rule

Ask about any form, office or rule in plain words — or say what to change. Every answer points to the page it came from.

Why is ENR-24817 not complete?

One thing blocks it and two need you. The programme authorization on page 1 is not signed or dated — until it is, no health information goes to the programme and nobody contacts Marisol. The form ticks ulcerative colitis but the code written, K50.90, is a Crohn's disease code. The member ID on the form has two digits swapped against the card. A request to Rosa Diaz at Lakeshore is drafted and waits for your approval.

Which offices send the most incomplete forms?

Four offices are below 65% complete on arrival this month: Riverbend Arthritis Center at 42% (old form version), Lakeshore Gastroenterology at 48% (patient authorization unsigned), Summit Rheumatology at 51% (prescriber signature missing) and Hill Country Digestive Health at 64% (representative relationship missing). Lakeshore's gap is almost always the patient signature.

What does the authorization need?

A valid programme authorization names the information shared, who shares it, who receives it, the purpose and an expiry, and carries the patient's — or a legally authorized representative's — signature and date. A representative must state their relationship. Until it is signed, the case stays Pending info and patient outreach is locked. Text messages also need the text-consent box.

How fast do forms become cases?

Median 38 minutes from receipt to case for complete forms this week, 41 over 30 days. Forms that need the office take a median of 19 hours, almost all of it waiting for the reply.

Questions

What patient services teams ask us.

How does it handle handwritten and faxed forms?

The Form Reader reads typed and handwritten fields and checkboxes from the brand's own form, and records the page, section and box of every value with a confidence. A value read below the confidence threshold — 90% by default — goes to a person to confirm before the case opens.

Does it check the patient authorization and consent?

Yes. The Completeness Checker tests that the programme authorization is signed and dated by the patient or a representative, that a representative states their relationship, and that text-message consent is recorded before any text is sent. Until the authorization is signed, patient outreach stays locked.

How does it check diagnosis codes and the prescriber NPI?

It checks that one diagnosis box is ticked and the ICD-10 code belongs to it — for example K51.x for ulcerative colitis — and that the prescriber NPI passes the check-digit test and matches the registry. It does not judge the diagnosis; when a code and a box disagree, the office is asked to confirm its own code.

What goes in the fax-back to the office?

Only what is missing or needs confirming, each with its page and section, plus the page to sign with the line marked and an e-signature link. No clinical advice and no full insurance numbers. The intake lead approves every request, and the follow-up timers start when it is sent.

Do people stay in control of what is sent and opened?

Yes. The agents read, check and draft; a person approves every request to an office and opens every case. Consent questions go to the privacy lead, and new or changed form rules go live only after the privacy lead approves them.

Can we set our own form rules for each brand?

Yes. Each brand form version has its own rules — 15, 13 and 12 on the three sample forms — each with where it applies on the form and whether it blocks the case, asks the office, needs a person or is done automatically. A new rule can be tested on the last 500 forms before it goes live.

How long does it take to go live?

The Agentic Solution Engine builds and deploys it from your requirements and documents — your brand enrollment forms, form rules, letter wording and a sample of past packets — and it goes live once every quality gate has passed. We will walk you through it on your own enrollment forms first.

See it on
